Medical care is longer provided and the office is now permanently closed.
Care will need to be obtained from your primary care providers or by establishing care with an appropriate endocrinologist. Patient's designated primary care physicians have received up-to-date records and should be able to resume your care and, when needed, provide referrals and records to transition to another endocrinologist.
Chart requests will require a signed record release form and pre-payment by check of the legally allowable chart retrieval fees.
